Admissions Check List

The following papers must be on file before the student is able to attend school:

• Registration Fee & tuition payment according to FACTS guidelines.
• Registration Form
• Application
• Parent/Student Agreement (signed by parent and child if in grade 3-8)
• Volunteer Form
• Record Release Form
• Pastor's Recommendation
• Copy of immunization record
• Copy of state-certified birth certificate
• Health appraisal filled out by physician.
